  • 適應症
    狹心症(心絞痛)：包括血管痙攣性心絞痛、慢性穩定型心絞痛。高血壓：使用nifedipine短效劑型治療高血壓，可能會發生血壓驟降、反射性心搏加速而引起心血管併發症。僅在無其他適合療法時，才可使用nifedipine短效劑型治療高血壓，且不建議舌下投予nifedipine膠囊。
  • 常見副作用
    1.Nifedipine 有較強的血管平滑肌作用，因血管擴張易發生頭痛、潮紅等副作用。2.本藥很少出現副作用，如有的話，也只有發生在初期治療的時候，而且短暫、輕微，偶發症狀 可能是頭痛、暖熱感、眩暈和嘔吐。3.有嚴重腎功能障礙者，BUN及 Creatinine 會上升。4.過敏症：發疹、搔癢等現象。
  • 禁忌(症)
    1.本藥不可用於已知對於Nifedipine或其所含任何賦形劑過敏的病患。2.本藥不可用於心因性休克(cardiogenic shock)的病患。3.本藥不可與Rifampicin併用，因酵素的誘導作用可能會使Nifedipine無法達到有效的血中濃度。
  • 注意事項與警語
    PO,do not halve or grind into powder. Store at room temperature in a cool, dry place away from light.1.This medication enhances the effects of antihypertensive and beta-blocking agents.2.If symptoms worsen with calcium channel blockers, the medication should be discontinued immediately.3.Using short-acting Nifedipine to treat hypertension may cause a sudden drop in blood pressure, reflex tachycardia, and cardiovascular complications. Short-acting Nifedipine should only be used to treat hypertension when no other suitable treatment is available.4.For pregnant and breastfeeding women, Nifedipine should only be considered after a careful assessment of individual risk-benefit by a physician, and when no other medication is available or has failed.5.10% of patients using Nifedipine may experience mild to moderate edema due to arterial dilation, primarily in the lower extremities, which can be relieved with diuretics.6.The frequency of angina attacks may increase after discontinuing beta-blockers. Starting Nifedipine at this time may worsen the condition. The beta-blocker dose should be gradually reduced before use.7.A small number of patients may experience ischemic pain after taking this medication. If this occurs, discontinue use and consult a physician.8.Research results show that in rare cases, patients with severe obstructive coronary artery disease taking fast-acting medications containing Nifedipine may experience an increased frequency, severity, or duration of angina attacks, or even acute myocardial infarction. The mechanism of action is unclear. Fast-acting medications containing Nifedipine are not recommended for patients with clinically significant aortic regurgitation, unstable angina, or within 4 weeks of acute myocardial infarction.9.Sublingual administration of short-acting Nifedipine capsules is not recommended.
  • 藥品交互作用
    1.與H2 接受器拮抗劑併用，曾報告發生Nifedipine之血中濃度增加，但直到目前，並未出現明顯臨床反應。雖然大多數的研究顯示，對於 Digoxin 血漿濃度無顯著臨床影響，但仍須記住可能有相互作用產生。2.與 Quinidine 併用時，不論 Quinidine 之劑量高低，血漿中 Quinidine 量皆會受到抑制。3.使用本藥品時併服葡萄柚或葡萄柚汁時，應注意可能產生的藥品相互作用。

  • 懷孕分級
    C 動物試驗顯示對胎兒有不良反應，但沒有針對孕婦做對照組研究；或是尚未對動物及人體做過試驗。此類藥品只有在對胎兒潛在利益大於潛在風險的前提下才能用於孕婦。
